We are a family-centered therapy clinic offering a full range of service options tailored to meet the needs of children, adolescents, and adults.

We serve children and adolescents with a range of developmental delays and challenges that include, but not limited to:

  • Fine and gross motor delays

We also specialize in working with adults to address concerns related to those listed above.

Beth Osten & Associates Protocol

  1. If you are interested in pursuing an evaluation or services for your child, please contact the intake coordinator of the discipline/program you are interested in. See department listing to the right.

  2. After speaking with the intake therapist and the insurance coordinator, please download our intake documents and complete them. All intake forms should be completed prior to the first scheduled appointment.

  3. Please arrive fifteen minutes prior to your appointment time to allow for review of the intake packet by our staff.

  4. Bring or fax required prescriptions to 847.663.1022

Additional Notes, Please Read

Please note that we can be difficult to find because we do not have a street facing entrance! We are located in an office complex on Dundee Road, that can be entered via Dundee Road westbound or via Landwehr Road northbound & southbound. Our office is on the far North end of the complex.

Arrive Early with Documents - Plan on arriving fifteen minutes early for your first appointment so our staff can verify that all documents are completed, and so we can make a copy of your insurance card and ID/drivers license. Please remember that we must have a prescription to begin treatment, but that one is not required for evaluations. This initial prescription must be obtained by the client.

  • Please print out and complete the Intake Packet and Intake Questionnaire (above). Do this as early as possible so you can read it carefully and complete all necessary components.

  • There are a number of documents in the Intake Packet; the following documents must be completed for your initial appointment: Client Information Sheet, Waiver and Release of Liability, Clinic Policies, Privacy Policy, and Service and Payment Agreement. If necessary, the rest of the packet can be submitted on your second appointment. All of our documents require signatures from both of the child's legal guardians.
